New Immunization Information
The Ohio Department of Health recently released a new vaccine requirement for the 2019-2020 school year. This letter is to inform you of the requirements in time to make appointments and obtain the documentation before the first day of school next year.
Next school year all students must have a vaccination for meningococcal disease before entering the 7th grade as well as the Tdap vaccine. Both are required before the first day of school for your child entering the 7th grade. Please click on the link, print and return the bottom portion of this form to the Middle School Clinic by the first day of school 2019.
Also included in this new legislation is a second meningococcal vaccine required to be given before entering the 12th grade.
If you have any questions about what immunizations your child has already had, please contact your physician. We suggest taking this letter to your physician to avoid any confusion.
